2005 Scholarship Application
This year, the Northern California Chapter of The American Institute of Wine & Food (AIWF/NorCal) will award a number of scholarships. Students eligible for these scholarships must be citizens of the United States of America enrolled in extended degree or certificate courses (the equivalent of at least six months or two semesters) at Northern California institutions. The courses must be in programs of Hospitality (Hotel or Restaurant Management), Culinary Arts (Chef, Sous-chef, or serving staff), Viticulture (wine-grape growing or vineyard management), or Enology (wine making). Scholarships will be based on the applicants potential for professional contributions and success in the students chosen field and financial need. These awards will be made at the AIWF/NorCal Annual Meeting to be held in early 2006.

One 2005 Scholarship Application, to be completed by the applicant
Two copies of the Scholarship Reference Form, to be completed by faculty or administrators in your program
One Verification of Student Status Form, to be completed by the Registrar of your institution.
Scholarship applications and all supporting documents must arrive in our office by the close of business Friday, November 11, 2005. (Please mail at least a week before this deadline and DO NOT send the forms by any method that requires a signature.) Those selected to receive the scholarships will be notified in December 2005, well in advance of the Annual Meeting of the Chapter early in 2006, when the scholarship awards will be made.

Please fill in all spaces completely and respond to all the questions concisely. You may open this file in MS Word and complete the application on your computer. Or you can open the MSWord or PDF (Adobe Acrobat) file on your computer, print it on your printer, and complete it clearly by hand. You can also type your answers on your own separate answer pages, being sure to number your responses exactly as the questions are numbered. In any case be sure your teachers or administrators and the Registrar have clean copies of the Reference and Verification forms to fill out and mail.
